We follow the clinical practice guidelines set forth by the American Academy of Pediatrics, while understanding that each child is unique and develops in his or her own way.

Registration Information
Please bring your child's immunization record, a list of current medications, and other medical records. Bring your child's insurance card to each visit, and please change the primary care provider to our practice physicians (Dr. Jain, Dr. Senator). Print and fill out registration forms before the first visit (see link below). Please arrive a few minutes early for registration.
Patient Forms and Insurance
We accept most major insurance payers. We will confirm insurance participation when you register for an appointment. We are not participating in Medicaid panels at this time.
